Physicians

Physicians may send a Referral Form by fax to 905-273-9800. Alternatively, an Online Referral can be made directly and securely into the Centre’s electronic medical record. Both options are available below.

 

 

Individuals

Individuals can self-refer by clicking the link Book a Consultation and completing the form. If your physician sends a referral it will be covered by your provincial health insurance plan.
